JOB DESCRIPTION: The Eligibility Coordinator for the YES Waiver will handle eligibility from the interest list through enrollment. This position must be able to work independently, be extremely organized, able to handle multiple task and able to manage and meet deadlines. This position will be responsible for completing LPHA intakes, maintaining the interest list by date and time of incoming calls, complete the screening process, submit various forms to DSHS, schedule and complete eligibility protocols, notify families of results, complete Medicaid paperwork when needed, and other task as determined. This position will also accurately enter information into various databases. This position will also complete initial treatment plan for new program enrollee's.

COUNTY: Bastrop County, Caldwell County, Fayette County, Gonzales County, Guadalupe County, Lee County

CREDENTIALS: Minimum Qualifications: Advanced clinical licensure is required (LCSW, LPC or LMFT) prior to hire date. Must have excellent oral and written communication skills. Clinical experience providing counseling to children, adolescents, adults and families. Valid driver's license in good standing required. Must have a valid Texas Driver's License.

Preferred Qualifications: Preferred AND required education, experience, job knowledge, skills and abilities helpful in the performance of this position, as indicated below: Comprehensive knowledge of typical and atypical development. Comprehensive knowledge of and understanding of team and family dynamics. Ability to contribute as a leader and facilitator on teams which include parents, caregivers, and other professional staff. Ability to perform under minimal supervision with considerable requirements for initiative, discretion, independent thought, judgment and self-motivation in matters of significance to families as well as the agency. Well-developed interpersonal skills, oral and written communication skills, time management/organization skills, and basic computer skills. Ability to work sensitively with families from a variety of cultural orientations. Preferred - Ability to read, write and communicate in Spanish. (receive a stipend for a foreign language)
